Montefiore Health System provides care management for around half a million individuals in New York. In 2011, its accountable care organisation (ACO) was selected to be part of the national Pioneer ACO Model programme. It was charged with providing higher quality care while at the same time reducing expenditure through improved care coordination.

The programme required the organisation to report its performance on quality and patient satisfaction measures, as well as to prove the reduced costs. This meant that Montefiore ACO needed to turn huge amounts of raw data into robust and useful information. To do so, the organisation worked with 3M Health Information Systems. Together, the two organisations developed analytics which helped measure quality performance as well as the total cost of care.

This collaboration enabled Montefiore to show that it had improved quality and outcomes whilst reducing costs; it was also able to demonstrate that it had lowered preventable readmissions and preventable attendances to emergency departments.
