Shifting care to the community is one of the big bets of the 10 Year Health Plan. This has been backed by plans for the creation of an NHS ‘online hospital’ with first patients using the service in 2027. Patients will be triaged through the NHS App and allowed to book scans at local Community Diagnostic Centres.

The move sets the tone for further transformation for neighbourhood care with digital and data at the heart. NHS digital leaders and NHS England representatives explored how the NHS organisations can delivery such transformation and what comes next.

    Key discussion points included:

    • How to enable neighbourhood teams with digital tools
    • Data sharing, interoperability and standards
    • The new NHS online hospital for community care
    • Remote monitoring
    • Integration with the NHS App and Single Patient Record
    • Infrastructure and connectivity
