Somerset Integrated Care System: The route to a Single Active Medication List

Bringing medication information together from multiple sources is a well-trodden path delivered through NHS backed incentives to implement Shared Care Records across the country. However, utilising that data intelligently to enable clinicians to understand the information and interact with it remains an aspiration. Better and Somerset ICS are working in partnership to provide a Regional Medications Platform that will consolidate the medication information from different care settings into a single coherent patient record.

This webinar will highlight and discuss the enablers and challenges from a clinical, technical and operational perspective to delivering a single active medication list across a region and will demonstrate the clinical value that can be derived along the way, beginning with how medicines reconciliation can be made a seamless task using data aggregated from multiple care sectors.
