Hear from guest speakers about the national Blueprinting programme, digital maturity assessment for ICSs and how Blueprinting can help bridge capability gaps, plus a panel discussion with experts on the areas of focus for ICSs.
You will also hear from Humber and North Yorkshire ICB talk about their Implementing an EPaCCS (Electronic Palliative Care Co-ordination System) in Humber and North Yorkshire Blueprint. Blueprint description:An EPaCCS is enabling health and care providers in the Humber and North Yorkshire to share information about end-of-life patient preferences and care plans. The need for EPaCCS was identified by clinicians and carers in the Humber and North Yorkshire who wanted to be able to support patients and their families at this very difficult time. A patient’s preferences about where they want to be cared for, where they want to die and the decisions about their care can change very rapidly during this process, and the health and care professionals providing care wanted to make sure that they had a shared and accurate view of a patient’s wishes.
