Northamptonshire has chosen Graphnet Health to provide a shared care record for people living in the county.
Graphnet’s CareCentric software will be used to combine information between Northamptonshire’s whole care community, consisting of 69 GP practices, Northampton General Hospital NHS Trust, Kettering General Hospital NHS Foundation Trust, Northamptonshire Healthcare NHS Foundation Trust, Northamptonshire County Council Social Care, 111 and GP out-of-hours service providers, and East Midlands Ambulance Service NHS Trust.
The shared record will be integrated into existing core IT systems used by the various NHS and local government organisations to give a more complete picture of an individual’s needs.
In practice, care professionals will be able to quickly access a record, via Single Sign On and in patient context, to receive a broad range of patient information at the point of care.
Kirstie Watson, digital lead for Northamptonshire Health and Care Partnership, said: “We are delighted to be making progress on delivery of this programme.
“The benefits will be far reaching. For the care community, it will enhance the quality of the information available so we can provide faster, safer care. It also means that authorised clinicians and carers will have the right information where and when it is needed, which will in turn make their jobs easier. The time they save looking for a patient’s information can be used to focus on delivering more care to our citizens.”
Northamptonshire follows the likes of Greater Manchester which accelerated the deployment of the CareCentric-powered GM Care Record in June 2020.
CareCentric will support assessments, data collection, workflow and notifications across the county so that care is co-ordinated, planned and integrated effectively around each patient and tailored for their individual requirements. As well as benefiting from a more joined-up approach, patients will only have to tell their story once and won’t have to repeatedly answer the same questions about their care.
Further phases of the project will include the analysis of data from across the county, so care providers can better understand and manage the population’s health and care needs, and also target interventions at relevant individuals or groups to ensure they get access to the services needed.
