Staffordshire and Stoke-on-Trent Integrated Care System has incorporated remote monitoring capabilities from Docobo to its One Health and Care (OHC) platform, allowing clinicians to access data from remotely monitored patients.

The One Health and Care platform is a confidential digital shared care record for people living in Staffordshire and Stoke-on-Trent, bringing together data from across health and social care. Docobo’s remote monitoring solution can enhance out-of-hospital care, by allowing patients to be closely monitored from the comfort of their own home.

The capability will extend to patients registered at any GP practice in Staffordshire or Stoke-on-Trent. Additionally, if a patient seeks services at any organisation outside of the region, but within the OHC scope – such as Walsall Manor Hospital – remote monitoring information can also be accessed for that patient.

The deployment of the Docobo tech will help to support clinical decision-making about patients. Clinicians will have instant access to baseline readings from patients’ latest remote monitoring observations, including temperature, blood oxygen levels, blood pressure and any recorded symptoms.

Eighteen months ago, Docobo’s digital health platform, DOC@HOME launched a new remote monitoring approach for cardio-respiratory diseases, supporting clinicians to assess breathing rates virtually.

“By incorporating the Docobo solution into this record, it will ensure clinicians have as rich and timely access to information as possible to support them in providing the best care possible for our population.”

Following the implementation of the system in Staffordshire and Stoke-on-Trent, a pipeline of pathways and populations is set to be onboarded soon.
